Current Surgical technique for retinal detachment with PVR

Purpose: To present current surgical technique and our results in the treatment of retinal detachment complicated by proliferative vitreoretinopathy (PVR).

Methods: Current management of PVR including principles of treatment, overview of the most often used classifications and surgical steps were presented. Retrospective consecutive interventional case series of 50 eyes in 49 patients suffered retinal detachment with PVR grade ≥C 2 that were treated at the Department of Ophthalmology, University Hospital in Uppsala/Sweden in the period of five years (2006-2010) were reviewed.

Results: Retina was completely attached in 48 eyes (96%) after mean number of 2,6 repeated surgeries (1-6). Visual acuity improved by ≥ 2 lines at Snellen charts in 41 eyes (82%), remained unchanged in 8 (16%) and worsened in 1 eye (2%).

Conclusion: Current surgical treatment including small gauge (25) four port pars plana vitrectomy with bimanual dissection of all membranes, combined with phaco/PC IOL, encircling band and silicone oil tamponade for advanced cases (grade ≥C 2) is an efficient and safe method in the management of retinal detachment complicated by PVR.
